Informed Consent for Telehealth Services
By using Jamala Health's telehealth services, you acknowledge and agree to the following:
- Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other means between a provider and a patient who are not in the same physical location.
- The assessment made available on the Jamala Health website does not create a doctor-patient relationship between you and Jamala Health. Medical treatment is provided by affiliated licensed healthcare providers.
- Licensed clinicians retain the decision to prescribe medication to patients based on their medical judgment, informed by a telehealth consultation and your medical history.
- You have the right to withhold or withdraw your consent to telehealth services at any time without affecting your right to future care.
GLP-1 Medication Consent
If you are prescribed GLP-1 medication through our platform, you acknowledge that:
- Your eligibility for GLP-1 medication is determined by a licensed healthcare provider based on your individual health profile.
- The answers you provide in the eligibility assessment determine if you are screened out of eligibility. A clinician will meet with you to determine if you qualify for a prescription.
- All medications carry potential risks and side effects. Your provider will discuss these with you before prescribing.
- Compounded GLP-1 medications are produced in FDA-regulated facilities. Although these facilities are highly regulated, compounded medications are not FDA-approved or evaluated for safety, efficacy, or quality.
- The decision to use compounded drugs is guided by the licensed provider's medical judgment.
- Results from medications may vary and are affected by your adherence to the program and your clinician's recommendations.
- You should speak with your provider about the specific risks and benefits of any prescribed medication.
Your Responsibilities
- Provide complete and accurate medical information during your intake and consultations.
- Inform your provider of all medications you are currently taking, including over-the-counter drugs and supplements.
- Follow prescribed treatment plans and report any adverse effects promptly.
- Attend scheduled follow-up appointments and check-ins.
- Notify your provider of any changes in your health status.
